Insurance Plan Participation

In order to ensure accurate billing to your insurance carrier, please:

1. Bring your insurance card with you to each appointment.

2. Pay your co-payment and other payments, per your insurance contract, at time of service.

3. Contact our office if you have any questions regarding your account.

Waterdam Medical Associates participates will most major insurance plans. Please call the benefit plan coverage number listed on the back of your insurance card to verify your specific coverage.
