North Suburban Family Health Center

At North Suburban Family Health Center our staff is comprised of skilled physicians, experienced health professionals and dedicated support staff who work together as a team to care for you in a comfortable setting.

Our physicians provide comprehensive internal medicine, family medicine and geriatric care, evaluating and treating a range of medical problems in adults and children 13 years of age and older.

Our comprehensive care for older adults includes these extended care services:

On-site physician visits to nursing and personal care homes
Physician on-call 24 hours
Telephone contact with a registered nurse
On-site diagnostic testing
Referral to nursing agencies
Home visits for patients who are homebound

NCQA (National Committee for Quality Assurance) works with large employers, policy makers, doctors, patients and health plans to decide what is important, how to measure it and how to promote improvement. Physician practices who have met stringent criteria measuring health plan performance are given three-year certification and also receive financial rewards depending on their level of program attainment.

Developed by NCQA and the American Diabetes Association, this program utilizes evidence-based measures to recognize physicians who provide excellent care to their diabetic patients.
